Doctor of Osteopathic Medicine, Master of Biomedical Sciences, and Master of Public Health students can request withdrawal from their respective program using the UIW Graduate Withdrawal Request. 

Doctor of Osteopathic Medicine students can request readmission to the DO program using the UIWSOM DO Readmission Application. 

Master of Biomedical Sciences and Master of Public Health students can request readmission using the UIW Graduate Readmission Form.
